DESCRIPTION | Accident and incident investigation is an important topic in OHS, with the primary aim to identify the cause of such occurrences and prevent similar situations from re-occurring. This unit thus introduces participants to accident and incident investigation, its purpose and methodologies. The unit also tackles the development and implementation of corrective action and follow-up. Relevant case studies are presented throughout the unit. Study-Unit Aims: The study-unit aims to: - Introduce participants to occupational incident and accident investigation; - Discuss accident and incident investigation teams; - Present the steps involved in investigating an accident; - Discuss accident causation models; - Explain the development of corrective actions and follow-up following an investigation; - Illustrate best practices in writing-up investigation findings and suggestions. Learning Outcomes: 1. Knowledge & Understanding: By the end of the study-unit the student will be able to: - Specify and appraise the purposes and advantages of occupational incident and accident investigation during discussion and a final assessment; - Reflect and evaluate who should carry out the investigation and the make-up of investigative teams during critical discussion and a final assessment; - Critically analyse the steps involved in conducting such investigations, including the collection of information, its analysis and its documentation during a final assessment; - Evaluate the use of accident causation models and the investigation of root causes during class discussion and a final assessment; - Articulate and appraise the role of investigation in developing and implementing corrective action and follow-up during a final assessment; - Critically analyse examples of investigative and corrective methods best-practice in a variety of occupational settings by means of case-study discussion and a final assessment. 2. Skills: By the end of the study-unit the student will be able to: - Set-up and manage an accident or incident investigative team; - Collect and critically reflect upon evidence relevant to the investigation including material evidence and information by means of interviews; - Implement accident causation models and infer root causes of accidents; - Develop corrective actions and follow-up procedures to evaluate these corrective actions.
