DESCRIPTION The unit presents two clinical practice experiences each of 4 weeks duration. The rotations will be covered within Karin Grech Rehabilitation Hospital, Rheumatology, Nephrology, Community Pharmacy, and Pharmacovigilance. Each student will be attached with a preceptor who will meet each student for on hour per week. Group tutorials will be held weekly for the students to reflect on advanced clinical pharmacy practice from the different scenarios where the students will be practising.

At the end of each experience, two seminars of 3 hours each will be held where students will present a case presentation. The students will attend and verbally participate in all professional activities at the site. They will create and maintain a current patient problem list and complete medication lists for each assigned patient in order to prioritize the delivery of pharmaceutical care within the setting. Students will design, recommend, monitor and evaluate a pharmacological plan based on each patient's problem list including tailoring of dose and dosage regimen, recommending appropriate alternative therapies and monitoring laboratory parameters as applicable.

Study-unit Aims:

- To participate in daily activities within a pharmaceutical healthcare service (hospital service, ambulatory care, community service);
- To develop skills in managing and improving the medication-use process;
- To identify opportunities for pharmacist intervention in patient care;
- To experience evidence-based, patient-centred medication therapy management within interdisciplinary teams;
- To develop leadership skills in pharmaceutical service development.

Learning Outcomes:

1. Knowledge & Understanding:

By the end of the study-unit the student will be able to:
- Demonstrate an understanding of the principles of application of pharmacotherapy in the management of minor ailments and chronic disease states;
- Identify risks and benefits of specific pharmacotherapeutic approaches in complex patient scenarios.

2. Skills:

By the end of the study-unit the student will be able to:
- Collect and critically assess clinically relevant data to facilitate monitoring and management of drug therapy plans;
- Monitor and recommend adjustments to drug regimens in order to maximize therapeutic outcomes;
- Efficiently collect, analyse and apply required literature sources for the appropriate clinical management of patients.
