DESCRIPTION | The study-unit presents three patient-focused practice experiences each of 6 weeks duration. The rotations will be undertaken in community practice, hospital and patient safety settings. Each student will be attached with a preceptor who will meet each student for one hour per week. Upon completion of the unit, individuals shall be highly qualified independent practitioners able to provide optimal patient-focused pharmaceutical judgements that account for quality, safe, effective and rationale use of medications. They can practice as integral members of an inter-disciplinary care team and be skilled at liaising with other healthcare professionals about medicinal products. These individuals will take a leadership role in quality and safety improvement initiatives with respect to medication use processes. Study-unit Aims: - To develop competent advanced clinical pharmacy specialists who are able to meet the needs of individual patients with complex pharmacotherapeutic requirements while acting as a fully integrated member of an interdisciplinary healthcare team; - To provide a hands-on exposure to a wide variety of complaints and illnesses and associated management in order to ensure broad foundational evidence-based pharmacotherapy knowledge and skills; - To provide experiences which develop skills in teaching other health care professionals and those in training to become healthcare professionals. Learning Outcomes: 1. Knowledge & Understanding: By the end of the study-unit the student will be able to: - Adopt problem-solving strategies in the management of a broad-range of disease states; - Determine from retrieved biomedical literature appropriate information to address requests for drug information; - Explain security and patient protections as related to the use of information technology to patient records; - Understand impact of different pharmacy procurement processes on medication-use processes; - Manage use of investigational medicinal products according to regulatory requirements and protocols. 2. Skills: By the end of the study-unit the student will be able to: - Identify opportunities for improvement of medication-use systems; - Design and implement quality improvement changes in medication-use systems; - Provide patients and health care professionals advice and information; - Document pharmaceutical processes including adverse drug reactions and medication errors; - Exercise leadership and practice management; - Demonstrate project management skills; - Function as a primary preceptor for students; - Use evidence-based literature to develop informed pharmaceutical assessment in drug use evaluation, patient therapy and population health risk.
