Further information shall be communicated to the parents/guardians of the successfully registered applicants via email closer to the start date of the summer bootcamp.
- Only ONE registration form per child must be filled in and submitted.
- UM is the Data Controller who is responsible for the personal data processed through the application for the Summer Camp and its related services.
- UM regards each child’s privacy as important and hence, it will seek to comply with the provisions of the Data Protection Act, Chapter 586 of the Laws of Malta and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) and will ensure that:
i. Personal data is processed fairly and lawfully.
ii. Personal data is processed with good practice.
iii. The collected personal data has been collected for the specific, explicitly stated and legitimate purposes indicated.
iv. The collected personal data will not be processed for any purpose that is incompatible with that for which the information has been collected.
v. The collected personal data will not be kept for a period longer than is necessary, having regard to the purposes for which it is processed.
- By submitting this registration form, the parent/guardian acknowledges that UM will have access to all the provided data and subsequently consent to such use.
- Money paid to the UM for this service, even if not utilized cannot be reimbursed in case that the child does not attend.
- While understanding that you would be contacted by the UM personnel as soon as reasonably possible regarding any emergency involving your child, you are giving your permission for your child to be transported to hospital or any other emergency medical facility in the occurrence of a serious illness or injury, whereas an ambulance or rescue squad members would be authorized to administer all necessary treatments. You are also giving your authorization for warranted health practitioners to examine and provide the necessary emergency medical treatment to your child in your absence.
- Should a child require a Learning Support Educator (LSE) and/or a nurse, the parents/legal guardians applying on behalf of the child are responsible for procuring such LSE and/or nurse for the entirety of the duration of the Summer Camp. The expenses incurred with respect to procurement of the same shall be reimbursed by UM upon proof of receipts and invoices.
- Only the persons listed in this registration form are authorized to pick up the child from the Summer Camp. Additional persons or changes are to be communicated to the Summer Camp Co-Ordinator in writing together with copies of their identity cards.
- Parents/guardians of children who exhibit continuous extreme challenging behaviour may be contacted and requested to attend to the needs of their child. Failure to do so may result in termination of service without refund.
- If the number of children registering for the Summer Camp is reached then the registration period will be stopped immediately. The Summer Camp is offered on a first come, first served basis.
- The Summer Camp will not take place if there are not at least twenty (20) children participating in such Summer Camp.
- Service may be suspended if not all the necessary documents are submitted to the Summer Camp Coordinator.
